Le design de l’\u00e9tude Praetorian DFT est pr\u00e9sent\u00e9 \u00e0 l’EHRA 2023. En effet, il a \u00e9t\u00e9 montr\u00e9 qu’il n’y a pas de b\u00e9n\u00e9fice du test de d\u00e9fibrillation en routine pour les DAI transveineux. Le test de defibrillation (DFT) reste recommand\u00e9 au cours de l’implantation de DAI sous-cutan\u00e9 m\u00eame si sa valeur ainsi que sa s\u00e9curit\u00e9 restent d\u00e9battues. Le score Praetorian a \u00e9t\u00e9 propos\u00e9 comme une alternative permettant de pr\u00e9dire le risque d’\u00e9chec de d\u00e9fibrillation. Ce score est utilis\u00e9 \u00e0 partir de donn\u00e9es obtenues sur la radio de thorax post implantation. Un score \u2265 90 est associ\u00e9 \u00e0 un risque d’\u00e9chec interm\u00e9diaire et un score\u2265 150 avec un risque d\u2019\u00e9chec \u00e9lev\u00e9.<\/p>\n

L’\u00e9tude Praetorian DFT est une \u00e9tude prospective randomis\u00e9e en 1:1 de non-inf\u00e9riorit\u00e9 multicentrique (37 centres) comparant les deux bras : un bras avec test de d\u00e9fibrillation\u00a0 et un bras sans test syst\u00e9matique mais avec calcul du score Praetorian. Dans ce bras, le score Praetorian est calcul\u00e9 apr\u00e8s implantation \u00e0 partir des donn\u00e9es de la radio et en cas de score > 90, un test de d\u00e9fibrillation est r\u00e9alis\u00e9. Le crit\u00e8re de jugement principal la survenue du premier \u00e9chec de choc pour traiter un \u00e9pisode de TV\/FV au cours du suivi. 965 patients doivent \u00eatre inclus et suivis pendant une dur\u00e9e m\u00e9diane de suivi de 40 mois. Les crit\u00e8res d’inclusion sont les suivants : patients \u2265 18 ans ayant une indication de DAI selon les recommandations et adress\u00e9s pour implantation de DAI sous-cutan\u00e9. Les patients avec un thrombus intra-atrial, une fibrillation atriale mal anti-coagul\u00e9e ou une autre contre-indication au choc \u00e9taient exclus, ainsi que les patients ayant une dur\u00e9e de vie > 12 mois, implant\u00e9s de LVAD ou pr\u00e9vus pour une transplantation cardiaque.<\/p>\n

Les r\u00e9sultats de l’\u00e9tude seront pr\u00e9sent\u00e9s en 2025. La validation prospective du score Praetorian sera pr\u00e9sent\u00e9e \u00e0 l’HRS en 2023.<\/p>\n
