En 2021, une m\u00e9ta analyse (15 \u00e9tudes, 3718 patients) men\u00e9e par Ravi et al. a conclu que, l\u2019ablation de Fibrillation atriale (FA) par radiofr\u00e9quence haute puissance courte dur\u00e9e, \u00e9tait plus efficace qu\u2019une ablation conventionnelle avec une r\u00e9duction de la dur\u00e9e de proc\u00e9dure et une s\u00e9curit\u00e9 comparable.<\/p>\n

Objectif de l\u2019\u00e9tude\u00a0:<\/u><\/strong><\/h3>\n

Comparer 2 strat\u00e9gies d\u2019ablation de FA\u00a0en termes de s\u00e9curit\u00e9 (en particulier sur l\u2019\u0153sophage) et d\u2019efficacit\u00e9 (taux de r\u00e9cidive), de mani\u00e8re cette fois ci randomis\u00e9e\u00a0:<\/p>\n

  • Strat\u00e9gie\u00a0conventionnelle : ablation point par point avec des tirs de 40 W sur la paroi ant\u00e9rieure et 25 W sur la paroi post\u00e9rieure.<\/li>\n
  • Strat\u00e9gie haute puissance-courte dur\u00e9e\u00a0: tirs de 70 W pendant 9 sec.<\/li>\n<\/ul>\n

    M\u00e9thodologie et population \u00e9tudi\u00e9e\u00a0:<\/u><\/strong><\/h3>\n

    Etude randomis\u00e9e, multicentrique ayant inclus 301 patients en Espagne, pr\u00e9sentant une FA paroxystique ou permanente (154 dans le bras ablation conventionnelle vs 147 dans le bras Haute puissance-courte dur\u00e9e).<\/p>\n

    Une fibroscopie oeso-gastroduod\u00e9nale et une IRM c\u00e9r\u00e9brale \u00e9taient r\u00e9alis\u00e9es dans les 24 \u00e0 72h post ablation.
    \nLes patients \u00e9taient suivis en consultation et devaient envoyer un enregistrement de leur rythme cardiaque tous les jours et en cas de sympt\u00f4me.<\/p>\n

    • Taux de r\u00e9cidive d\u2019arythmie identique dans les 2 groupes.<\/li>\n

    • Pas de diff\u00e9rence en termes de l\u00e9sions \u0153sophagiennes entre les 2 groupes.<\/li>\n<\/ul>\n

      – Les cath\u00e9ters d\u2019ablation utilis\u00e9s dans cette \u00e9tude ont montr\u00e9 leur efficacit\u00e9 et s\u00e9curit\u00e9 uniquement jusqu\u2019\u00e0 50 Watts et pas au-del\u00e0. Des cath\u00e9ters d\u00e9di\u00e9s \u00e0 la haute \u00e9nergie (tel que le QDOT) sont d\u00e9sormais disponibles sur le march\u00e9.<\/p>\n
